Our Expression of Wishes Form is designed to give you the opportunity to record your wishes for your final farewell, to allow your loved ones to say goodbye in a way that you want, and save them the heartache of having to make these decisions on your behalf at such a difficult time. Do consider your specific preferences, note them down, and file the form away with your important personal information.
